To receive the cash back you need to:

1. Purchase a Calphalon One Hard Anodized Cookware Set – 8 pieces or larger.
2. Cut out and send the original UPC bar code from the product packaging.
3. Include the original sales receipt. The receipt must be dated between 1/1/09 and 6/30/09.
4. Complete the mail-in rebate certificate and send it with the UPC bar code and the receipt to the CMS Rebate Center.
